The informal review of the request for appeal process is initiated by the Ambulance Service Provider filing a written, signed request for an informal review of the request for appeal with the Department’s Bureau of Professional and Ancillary Services or its authorized agent, within 90 calendar days after the date of service pursuant to 89 Ill. Adm. Code 140.491(j) .

PassPORT is a free web portal developed by First Transit for use with NETSPAP. It enables certain providers (currently Long Term Care Facilities, Dialysis Centers, and Non-Emergency Transportation (NET) Providers) to view the authorized, denied, and pending requests as stored in the ADEPT prior authorization system and to submit Single Trip and Standing Prior Authorization (SPA) requests.

PassPORT is available 24 hours a day, 7 days a week, 365 days a year and there is no limit on the number of transactions allowed. PassPORT allows Providers to organize and access Participant information and there is no waiting to talk to a Customer Service Representative.

To create a PassPORT account:

  1. Click on the “Request a New Account” link on PassPORT’s front page.
  2. Enter the information requested.  Once you select a Provider type, you will be asked to enter your Medicaid Provider ID. This is the 12-digit number you use to bill Illinois Medicaid.
  3. Click on the “Submit request” button. You will receive a response by email.
